The following restaurant inspection reports were provided by the New Mexico Environment Department. 

 

Los Alamos

 

American Legion Post #90, 1325 Trinity Dr.

Date inspected: April 26

Violations: One high-risk violation for poor personal hygiene — food handler had a drink at prep table. Drinking and eating cannot be done in food prep area. Corrected. One moderate-risk violation for contaminated equipment — seal around door of walk-in refrigerator needs repair. One low-risk violation for ventilation/lighting — hood/vent filters need cleaning to avoid potential fire. In back, icemaker is used for ice baths only, not potable.

Status of establishment: Approved, no follow-up required.

 

Central Avenue Grill

Date inspected: April 27, closing

Status of establishment: Facility has been closed.

Violations: One high-risk violation for contaminated equipment — Didn’t have sanitizer solution made for wiping towels. Corrected at 200 ppm. One moderate-risk violation for contaminated equipment — test strips needed.

Los Alamos Little League Peanut Gallery Snack Bar

Date inspected: April 27

Violations: One moderate-risk violation for improper holding — thermometers are not working. Need new thermometers.

Notes: Might need variance on equipment. All food handlers must wear hair restraints. Get new permit posted. All storage supplies and Cokes must be stored 6” off the floor. Person handling money cannot handle food items unless changing into gloves.

Violations: One high-risk violation for contaminated equipment — Dishwasher solution less than 50 ppm. Corrected to 100 ppm.

Santa Fe

 

El Chile Toreado, 950 Cordova Road

Date inspected: April 30

Violations: Two high-risk violations, one for inadequate cooking/cooling — beef cooked was not being cooled properly. Left at room temperature. Temperature was 154 degrees (put back on steam table); one for contaminated equipment — bleach (Clorox) used has other ingredients in it. Use only regular Clorox bleach. No deodorizer in it. Corrected. One moderate-risk violation for contaminated equipment — steam table surface, counter are messy with food debris. Clean. Hot water was available for three-compartment sink and the hand wash sink. One low-risk violation for ventilation/lighting — exhaust hood filters are greasy. Clean.

Notes: New under counter refrigeration unit has been bought and used exclusively for meat products, only.

Date inspected: April 20, pre-opening, opening

Violations: None

Mr. Polish, 1311 Siler Road

Date inspected: April 30

Violations: One high-risk violation for storage — bleach container was stored with case of sodas, corrected. Adequate hot and cold running water was inadequate for this unit. One moderate-risk violation for plumbing/waste disposal — the wastewater storage tank is not adequate. Water spilling out into the street. One low-risk violation for storage — cases of sodas were stored on the floor. Lift at least 6”.

Piñon Elementary School, 2901 Camino de Los Cabolero

Violations: One high-risk violation for improper holding — improper thawing process, corrected. One moderate-risk violation for improper holding — thermometers inaccurate, need to be calibrated. Corrected. One low-risk violation for storage — food stored on the floor. Corrected.

The Providers processor, 3134 A Rufina St.

Date inspected: April 26, complaint

Violations: Five high-risk violations, one for improper holding — packaging noodles cooked into clamshells; one for contaminated equipment — poison bait next to cooking oil in prep area, using pesticides not approved for food service; two for poor personal hygiene — no hand wash sink in facility, using three-compartment sink; employees drinking from bottles, water in prep area; one for plumbing/waste disposal — cross-connection on food prep sink (dirty). Two moderate-risk violations, one for improper holding — chef salad at 47 degrees, sesame at 47 degrees; one violation for animals/vermin/opening — garage door held open, restroom door not self-closing; ineffective pest control. Three low-risk violations for ventilation/lighting — lights not shielded, inadequate light in cooler; inadequate ventilation in room, hot; one for floors/walls/ceilings — ceiling inappropriate vinyl-covered fiberglass.

Notes: Mouse droppings in storage area, black light near water heater. Dead mice (two each) in tin cat trap on floor. Noodles cooked at 7 a.m.1050 noodles in walk-in, 45 degrees. 1100 noodles package holding on prep table, 57-62 degrees, product condemned. Pad Thai processing records, none on file. Batch code not firmly affixed. Processing unapproved products.

Notes: Items corrected, high-risk from previous inspection: hand sink in place and properly equipped; pesticides removed from premises, dead mice and bait/glue traps; process procedure for cooked items is outlined. Facility reviewed only for temp and process procedures. Approved for closed sandwiches. Not approved for chef salad with meat or burritos with meat. 

Violations: Food items not dated. Sandwich labels improper, no manufacturer. No batch coding system on file, not adequate. No recall procedure on file, not adequate. None on label, misbranded turkey and ham, no count.

Violations: Two moderate-risk violations, one for animals/vermin/openings — opening below garage doors and exterior doors; one for other — not washing cabbage before cutting. Corrected. One low-risk violation for contaminated equipment — openings above and beside walk-in cooler not easily accessible for cleaning. Also, a violation for contaminated equipment — inadequate space for clean to dishes to air-dry, employees having difficulty finding space.

Notes: Noodles cooked at 7 a.m., 67 degrees by 8:30 a.m. on baker’s rack and sheet pans less than 2” thick. Inadequate cooler space, walk-in packed, no walk-in space.
