Notes: The two hand wash sinks in the kitchen have been repaired. The one in the front area kitchen is not leaking anymore and hot and cold running water is provided all the time and the back one is also working properly. The fluorescent light bulb in the dry storage area has been replaced with a new one and adequate lighting is now available for that area.

Violations: Three high-risk violations, one for storage — improperly labeled cleaning container “sterilizer,” “409,” “Simple Green” reported to be a QUAT solution. Smell indicates otherwise; two for other — facility food service has exceeded limits of restricted menu permit. Only allowed to serve food in single-serving containers. Now holding food in bulk containers, multi-serving containers. Two moderate-risk violations for contaminated equipment — No sanitizer test kit, not testing; cleaning plastic bulk serving containers without proper ware wash facilities. Only inside components allowed under permit. Facility may submit plan for overnight bulk/multi-serving holding, in addition to single-serving process.
Notes: Not holding foods for 24 hours. Recommend that individuals that put partially serving containers, mark their personal food items on containers. Notice of major violation issued. Must respond within 10 days.

Must be sanded or get new ones. Three low-risk violations, one for floors/walls/ceilings — the kitchen walls by the back door are filthy. Also, the walls by the walk-in cooler area; two for ventilation/lighting — there is no light bulb in the True Refrigeration unit reach-in; the vent on the ceiling in dishwasher room is very greasy. Clean. Also, the ceilings are filthy. Clean. Exhaust hood filters in the grill area of restaurant are greasy.

Smith’s Food and Drug Center, 2308 Cerrillos Road
Date inspected: Oct. 18, complaint
Violations: Two moderate-risk violations, one for improper holding — the display refrigeration unit where mussels and other fish are stored did not have a thermometer. Provide thermometer for all the units; one for contaminated equipment — QAT test strips in three-compartment sink are wet and discolored. Provide a new one and make sure it is used to monitor the sanitizing solution for QAT in the three-compartment sink.
Notes: The source for these fresh mussels is from South Wind Foods, Vernon Calif. Regarding the complaint, store management told inspector that they only got one call from the complainant. No other person called regarding this.
