The following restaurant inspection reports were provided by the New Mexico Environment Department.

Los Alamos

Don Quixote Vanilla, 236 Rio Bravo
Date inspected: Oct. 3
Violations: One low-risk violation for poor personal hygiene — must wear hair restraints.

El Rigoberto’s Taco Shop, 166 Central Park Square
Date inspected: Oct. 3, opening
Violations: One moderate-risk violation for contaminated equipment — seal around walk-in refrigerator may need replacing. Two low-risk violations for floors/walls/ceilings — ceiling panel missing, will put in today; self-closing devices for men’s restroom needed.

Date inspected: Oct. 4, follow-up
Violations: Two high-risk violations for contaminated equipment — scoop for ice was laying on top of ice machine. Corrected. Placed in container after washing it; sanitizer > 200 ppm. Corrected.  
Notes: Thermometer read 142 degrees on green chile. Inspector’s thermometer read 147 degrees. All food handlers must wear caps and gloves.

Home Run Pizza, 1627 Central Ave.
Date inspected: Oct. 3
Violations: Four high-risk violations, two for improper holding — Below cheese held at 61 degrees. Corrected. Two for contaminated equipment — food handler has bare hand contact with ready-to-eat foods. Had him  throw away the salad, tomato and cucumber; slicer has food product on it. Must clean after each use. Corrected. One moderate-risk violation for improper holding — need thermometer inside all refrigerators and freezers. Two low-risk violations, one for poor personal hygiene — food handlers must wear hair restraints. One for floors/walls/ceilings — wall behind prep area must be smooth, non-absorbent and easily cleanable.
Notes: Do not use bottom portion of prep refrigerator. Long prep refrigerator is working on top, but not bottom. Must get serviced.

Corrected. One moderate-risk violation for contaminated equipment — cutting board replaced. Corrected.

Violations: One high-risk violation for improper holding — prep refrigerator, cheese at 66 degrees. Owner said the refrigerator hasn’t been repaired, but is using it for lunch only. Hand wash sink, no towels. Corrected.
Notes: The prep refrigerator has been condemned. Cannot use until serviced and reinspected. A notice of violation will follow this inspection.

Violations: One high-risk violation for poor personal hygiene — hand sinks in the kitchen are not working properly. One has the cold running water turned off because it leaks continually. The other hand sink at the back, by the three-compartment sink has all faucets turned off. These sinks must be fixed within 10 working days, by Oct. 15. One low-risk violation for ventilation/lighting — the fluorescent light bulb in the dry storage area is burnt. The area is dark. Replace light bulb as soon as possible.

Four low-risk violations, one for administration — food establishment permit posted has expired; two for floors/walls/ceilings — the floor in the dry storage room outside has the vinyl tiles peeling and loose. Also, the floor under the hand sink in the kitchen is filthy, with the grease trap container spilling grease. Please scrape and clean this floor. The ceiling by the dishwasher area has dust. Clean; one for ventilation/lighting — two light bulbs in the kitchen area do not have shields.

Violations: Seven high-risk violations, one for contaminated equipment; three for poor personal hygiene and three for storage. Eight moderate-risk violations, one for improper holding; five for contaminated equipment; one for storage and one for other. Ten low-risk violations, one for contaminated equipment, one for animals/vermin/openings; two for floors/walls/ceilings; four for storage; and two for ventilation/lighting.
Violations at
Paisano’s deli:
• Seal back splash to wall on sides and top. Clean more frequently.
• Rellenos held unprotected atop display case. Not covered.
• No trashcan near hand sink.
• Uncovered foods held uncovered on bottom shelf of table.
• Using wash sink to do food wash with dirty dishes in the next compartment.
• Inadequate space for dirty or clean dishes.
• One pot of jalapeños held on floor.
• Prep area ceiling not smooth. Does not meet requirements.
• Prep area lights not shielded.
• No food thermometer available to monitor temps. Corrected.
• No food prep sink, not washing limes before cutting.
• No sanitizer made for use.
• No sanitizer test kit.
• Employee handled raw meat with gloves and never changes gloves afterward to serve food to customer or after performing other tasks.
• Opening in ceiling above vent hood. Ceiling tile missing. Install tile.
• Open space above vent hood. Seal area from hood to ceiling.
Violations at Paisano’s bakery:
• Toxic chemicals (WD-40 and lube oil) stored above clean aprons and towels.
• Unmarked bottle in wash/prep area.
• Equipment mixers, tables and bakers pans not cleared daily/sight and touch.
• Bakers pans with flour and cloth liner not cleaned.
• Several food containers left open, coconut and pecan icing, etc.
• One bucket marked Bavarian crème filling open, but contained brown liquid. Container mislabeled.
• Unused equipment held in prep area, remove from facility if not used, or keep covered, or keep clean. Clean mixer bowl held near and below trashcan.
• No paper towels in men’s toilet room.
• No hot water in toilet rooms. Ran water for two minutes. Must provide hot water at 100 degrees or more.
• Clean pot lids and paddle held in contact with floor.
• Conduit left exposed, not clean, not easy to clean. Either clean or recess or replace with NEMA fixtures.
Violations at
Paisano’s meat market:
• Inadequate light in cooler.
• Hand sink not in convenient location.
• Wall below ware wash sink has particulate accumulation.
• Not testing sanitizer solution.
• Liner papers not held protected from contamination.
• Lights in prep area not protected.
 Notes: One high-risk violation for contaminated equipment; three for poor personal hygiene; and three for storage were corrected on site. Must send fried pork samples to local food lab for water activity analysis. They will help establish a proper holding temp for such deep fried food items.
