The following restaurant inspection reports were provided by the New Mexico Environment Department.

Betty Ehart Senior Center, 1000 Oppenheimer
Date inspected: July 27
Violations: One high-risk violation for contaminated equipment — sanitizer at 10 ppm, not strong enough. Corrected.

China Moon, 121 Central Park Square
Date inspected: July 27
Violations: Four high-risk violations one for improper holding — eggs held at 75 degrees. Corrected, tossed. One for contaminated equipment — scoop handle lying down on rice in rice bin. Corrected. Sushi rice needs to be in enclosed bin. Three low-risk violations, one for animals/vermin/openings — outside back door must be kept closed. Two for floors/walls/ceilings — floor tiles need replacing; floors, walls, ceilings must be kept clean at all times.

Sombrillo Nursing and Rehab, 1011 Sombrillo Court
Date inspected: July 27
Violations: Two high-risk violations, one for poor personal hygiene — hand wash sink has no paper towels or sanitizer. Corrected. One for plumbing/waste disposal — floor drain for vegetable sink needs two-inch air gap. One low-risk violation for floors/walls/ceilings — tiles in dining room need to be replaced where damaged.

Subway, 745-B N. Riverside Dr.
Date inspected: July 25
Violations: Two high-risk violations, one for contaminated equipment — improper washing, rinsing and sanitizing. Observed ladle in hand wash sink, observed male worker rinse tray/bin and put back in use without washing, rinsing and sanitizing. Corrected. Manager advised to inform staff. One violation for plumbing/waste disposal — faucet leaks at three-compartment sink, drain leaks. Keep container underneath to prevent puddling on floor until repaired. Men’s toilet running continuously. Needs immediate repair. Two moderate-risk violations, one for improper holding — all refrigeration must have thermometers, none observed, only indicating thermostat. Need thermometer units in all refrigeration, cold-holding equipment. One for contaminated equipment — cutting boards are scarred up. Needs to be sanded, resurfaced or replaced to keep sanitary. Three low-risk violations, two for floors/walls/ceilings — floor tiles in disrepair, need repair to prevent accidents and to keep floor sanitary; cobwebs present above soda equipment in/near office. One for ventilation/lighting — protective cover missing on light bulb in walk-in freezer. Must be able to prevent broken glass from light bulb falling on foods.

Date inspected: July 25
Violations: One high-risk violation for improper holding — hamburger meat defrosting at room temperature. Must keep in refrigeration until use. Corrected. One moderate-risk violation for storage — unlabeled meats out of their original packages. Need to label i.e. beef, pork, chicken, etc. Date, etc. One low-risk violation for ventilation/lighting — covers on light fixtures missing. Need to replace to prevent bulbs from breaking an spreading glass onto foods, food surfaces. Trash bin kept locked, good.

Date inspected: July 24
Violations: One high-risk violation for improper holding — three one-quart containers, chicken prepared July 23 and held at 41 degrees; three-gallon green chile prepared July 23 and held at 45 degrees; ground beef held at 45 degrees. All items destroyed, corrected.
Notes: Provided training on danger zone, cooling food, reading, care and use of thermometer, dating of foods. Facility will maintain temperature charts for submission via fax.

Two moderate-risk violations, one for improper holding — bar under counter refrigeration units do not have thermometers; one for contaminated equipment — chlorine test strips are not available, corrected. Dishwashing sanitizing solution was good at 100 ppm. Three low-risk violations, one for floors/walls, ceilings — the wall by the dishwasher is filthy, clean. Two for storage — boxes of tomatoes were stored on the floor, corrected; employee jacket was hanging on the dry storage food shelf.
