The following restaurant inspection reports were provided by the New Mexico Environment Department.

Date inspected: July 5
Violations: Two high-risk violations, one for contaminated equipment — non-approved poison bait in prep area, corrected; one for storage — cooler drain not shielded. Three moderate-risk violations, two for contaminated equipment — no sanitizer test kit for chlorine, not testing dishwasher, QUAT test kit was water damaged, not usable; bare wood sides on wood shelves; one for animals/vermin/openings — no effective pest control, mouse (snap type) in facility. Three low-risk violations for floors/walls/ceilings — no base coring in several places; walls and ceilings improper, non-durable, openings, peeling surfaces.
Notes: Facility has planned remodel, plans to be submitted.

Violations: One moderate-risk violation for improper holding — install thermometer inside freezers. Two low-risk violations, one for floors/walls/ceilings — floors wet in some areas of the kitchen; one for storage — food-grade oil stored close to the floor.

Violations: Six high-risk violations, three for improper holding — front display cooler and reach-in refrigerator not holding food at below 41 degrees; food items held out of temp in display case; cakes held unprotected from consumer, uncovered, atop counter; one for poor personal hygiene — dishwasher drinking from open cup in wash area held on shelf with clean dishes, two cups found and removed, employee admonished; one for storage — fly strips in prep and wash areas. Three moderate-risk violations, one for improper holding — no food thermometer presented for use; one for contaminated equipment — dirty pot in prep area floor, inadequate dirty dish storage; one for animals/vermin/openings — doorways open, no screening flies indoor and birds outdoors, doorways must be screened to protect against entry of birds, insects and pests. Two low-risk violations, one for storage — Fly strips in prep and wash areas; two for ventilation/lighting — unprotected lights in wash area, one fixture.
Notes: Facility must have a person in charge at all times of operation. No one accepted the role when questioned. Everything on display cooler must be at temperature or kept on ice.

Date inspected: July 6, follow-up
Violations: One high-risk violation for improper holding — carne asada was held at 50 degrees. Destroyed, corrected. Keep temperature logs for all food items and fax daily to NMED. Required to have digital thermometer.

Date inspected: July 5, opening
Violations: Three high-risk violations, two for poor personal hygiene — complete installation of hand wash sink, completed; install second hand wash sink within 30 days; one for storage — meats stored over ready-to-eat foods. Corrected. One moderate-risk violation for improper holding — replace curtain inside walk-in box. One low-risk violation for floors/walls/ceilings — seal rear door.
Notes: Dishwasher and wiping cloth sanitizer is good. Sanitizer test strips are on site.

Date inspected: July 6, complaint
Violations: One high-risk violation for contaminated equipment — wiping cloths unsanitary. Corrected. One moderate-risk violation for storage — unlabeled containers in the kitchen. One low-risk violation for storage — food stored on the floor.
Notes: Inspected, no problem regarding complaint found during inspection. All temperatures were within required range. Complaint closed.

Tortilla Flats, 3139 Cerrillos Road
Date inspected: July 5, follow-up
Violations: One high-risk violation for improper holding — beef in cooler at 48 degrees, item destroyed, corrected. One low-risk violation for ventilation/lighting — inadequate lighting in walk-in cooler. 8/215 lux required.
Notes: both hand sinks operational, adequate hot/cold water. Food items dated. Four items dated incorrectly, corrected. Front hand sink, food and utensils stored too close, encroaching on adequate space, trash can potentially encumbers access. Ware wash and ceiling-roof access port, door not sealed. Cooling of foods must be verified using a food thermometer. NMED suggests use of cooling charts, will be required if future violations.
