Violations: Two high-risk violations, one for contaminated equipment — non-approved poison bait in prep area, corrected; one for storage — cooler drain not shielded. Three moderate-risk violations, two for contaminated equipment — no sanitizer test kit for chlorine, not testing dishwasher, QUAT test kit was water damaged, not usable; bare wood sides on wood shelves; one for animals/vermin/openings — no effective pest control, mouse (snap type) in facility. Three low-risk violations for floors/walls/ceilings — no base coring in several places; walls and ceilings improper, non-durable, openings, peeling surfaces.
Notes: Facility has planned remodel, plans to be submitted.

Date inspected: July 6
Violations: One moderate-risk violation for improper holding — install thermometer inside freezers. Two low-risk violations, one for floors/walls/ceilings — floors wet in some areas of the kitchen; one for storage — food-grade oil stored close to the floor.
Status of establishment: Approved, no follow-up required.
