Date inspected: June 8
Violations: Two high-risk violations, one for poor personal hygiene — hand sink without soap and two without trash cans nearby. Corrected. One violation for storage — Rice Krispie treats held unprotected for consumer self-service. Corrected. Four low-risk violations, one for contaminated equipment — openings to crawl space above and below rooms not sealed/open; one for floors/walls/ceilings — base coring not installed in prep area; two for ventilation/lighting — one light inoperable; one light unprotected above salad station.

Violations: One high-risk violation for improper holding — creamers held at 70 degrees. Corrected. One moderate-risk violation for improper holding — inaccurate thermometer inside serve line refrigerator. Please check refrigerator for proper operation. One low-risk violation for ventilation/lighting – light out of order in kitchen.

Violations: One high-risk violation for improper holding — beans cold holding at 47 degrees. Corrected. Items destroyed. CAR corrections response required by June 15. Must provide written cooling procedure to NMED for all food items cooked and cooled on site. Two moderate-risk violations for contaminated equipment — not monitoring food temps; not testing dishwasher chlorine sanitizer. Three low-risk violations, two for floors/walls/ceilings — walls peeling in some areas, prep and wash; backsplash not sealed to wall.

Violations: One moderate-risk violation for animals/vermin/openings — pest control measure not effective. Pigeons, outdoors. Submit written plan to address above cooler, and open area in back by June 15. One low-risk violation for ventilation/lighting — light protection cracked (diffusers) missing in wash area. Recommend better ventilation in wash area.

Violations: Three high-risk violations, one for unsafe sources — beans in refrigerator not dated; one for improper holding — chicken at 55 degrees; one for poor personal hygiene — hand sink not tightly mounted, nor is faucet. Three moderate-risk violations for contaminated equipment — no sanitizer test kit; only two stoppers for three-compartment sink. Three low-risk violations, two for floors/walls/ceilings — base coring not installed; opening into wall and its interior. One for ventilation/lighting — one light in hand wash area not working.

El Gancho Fitness, Swim and Racquetball Club Snack Bar, 104 Old Las Vegas Hwy.
Date inspected: June 7
Violations: One moderate-risk violation for improper holding — one thermometer is out of calibration. Required to have digital thermometer on hand until proper calibration is achieved. Sanitizer properly labeled, 200 ppm.

El Gancho Fitness, Swim and Racquetball Club, 104 Old Las Vegas Hwy.
Date inspected: June 6, follow-up
Violations: One high-risk violation for improper holding — ground beef at 47 degrees; chicken at 45 degrees. Two moderate-risk violations, one for improper holding — all four thermometers 10 degrees out of calibration. All calibrated to 32 degrees in ice/water. Quat sanitizer not labeled. One for contaminated equipment — only one stopper for three-compartment sink.
Notes: Repeat violations may result in permit suspension. Will follow-up to verify corrections of items. Manager to submit all food thermometers to NMED SFFO at 9 a.m. June 7 with a bag of ice for calibration and written explanation for out of calibration incidents. Do not hold any meats out of temperature at follow-up.

Lan’s Vietnamese Cuisine, 2430 Cerrillos Road
Date inspected: May 31, complaint
Violations: One moderate-risk violation for animals/vermin/openings — roaches were seen in kitchen and some dining areas.
Notes: Facility owners voluntarily want to close and have this facility sprayed. Closed from June 1-June 3. Restaurant will be reinspected on June 4.

Lucky Bean Café, 500 Montezuma St., Suite 408
Date inspected: June 6
Violations: One high-risk violation for poor personal hygiene — hand sink is used to store things, also food debris pieces were found in there. Use only for hand washing. This was corrected during inspection. One moderate-risk violation for contaminated equipment — towels look so discolored with coffee stains. Replace more often and use bleach to soak more often. The three-compartment sink is not in use for sanitizing, but chlorine and test strips area available.
