The following restaurant inspection reports were provided by the New Mexico Environment Department.

Española

Arby’s, 931 N. Riverside Dr.
Date inspected: May 9, complaint
Violations: One high-risk violation for improper holding — meat product held at 123 degrees, corrected by disposing. One moderate-risk violation for other — walls/ceilings need repair/maintenance.
Notes: Dry storage overall OK, clean. No chemicals stored in food prep area. Staff wearing clean uniforms, observed good sanitary skills. Facility needs maintenance, i.e., paint, replacement of ceiling tiles, etc. Permit missing. NMED will research expiration date and provide copy.
Status of establishment: Approved, no follow-up required.

Blake’s Lotaburger
#18, 914 Riverside Dr.
Date inspected: May 8
Violations: Two moderate-risk violations for animals/vermin/openings — insect control unit installed over a food prep unit area. Potential to cross-contaminate food with dead insects. Need to relocate to other location, i.e., hallway where no food is stored underneath. Major notice, corrective action issue. Provide NMED with letter of proof that unit has been moved ASAP — within three days, due on May 11. Screen door doesn’t seal properly, cracks allow access to flies and other insects and possibly other vermin, i.e., mice. Need to seal. Insect control unit, Vector Plasma, needs to be relocated to non-food prep area.
Note: Facility drastically needs refrigeration for veggies/condiments prior to next inspecting. Holding food on ice is not working. Onions are at 42-44 degrees. Need to replace current system with proper refrigeration.

Center Market Bakery, 745 N. Riverside Dr.
Date inspected: May 8
Violations: One high-risk violation for poor personal hygiene — towels missing at hand wash sink. Corrected, as they were replaced. One moderate-risk violation for contaminated equipment — sanitizer test kits needed to test sanitizer frequently.
Notes: Hot water not prevalent during inspection, only luke-warm. Need to ensure ample supply of hot water always. Please look at maintenance of hot water supply. Cake refrigerator at 56 degrees, move all foods to walk-in until display unit can hold foods at 41 degrees or less. Three-compartment sink air gapped. Staff wearing hair nets, clean uniforms. Cake display fridge needs service to hold foods at 41 degrees or less. Hot water insufficient, adjust/replace water heater to have plenty of hot water at all times.

Center Market Meat Market, 745 N. Riverside Dr.
Date inspected: May 8
Violations: One high-risk violation for poor personal hygiene — no hot water present. Need to repair hot water heater so hot water is always present and in ample supply. Major notice of violation issued. Repair prior to operating, provide written proof of correction of violation. One moderate-risk violation for contaminated equipment — test kits missing, sanitizer at 200 ppm. Need to have test kits to ensure that sanitizer is between 50-200 ppm. Two low-risk violations for floors/walls/ceilings — floors dirty and bloody. Need to be cleaned more frequently. Walls are dirty, dusty and moldy. Need thorough cleaning. Ceiling missing tiles, dirty. Needs more cleaning.
Note: Major notice of violation issued for lack of hot water at hand/three-compartment sink. Proof of written correction needed by May 11.

Date inspected: May 9
Violations: One moderate-risk violation for other — walls/ceilings need repainting; appears to have been started, but not completed.
Notes: Floors overall clean. Dry storage, overall OK. No chemicals present in food prep/service area. Staff wearing clean gloves, uniforms, hair restraints — good job. Facility continues to improve from last year. Really good job, ladies.

Santa Fe

Bench Warmers, 205 Caja del Rio

Status of establishment: Closed. Temporary seasonal applications requested.

Burro Alley Café, 207 W. San Francisco St.
Date inspected: May 8, follow-up
Violations: One low-risk violation for ventilation/lighting — the exhaust hood filters are very greasy. Clean.
Notes: The prep counter close to the hand sink has been moved further away from the hand sink. The walls by the prep counter table have been sealed. All violations noted during the regular inspection have been corrected.

Notes: Walk-in cooler air temperature 35 degrees, undercounter refrigeration unit air temp, 32 degrees. Okay to operate, issue permit. Equipments all meet the NSF standards.

Violations: None
Notes: All foods prepared at Larragoite Elementary School and served at Eldorado Elementary. Fully pre-packaged.

Violations: One high-risk violation for contaminated equipment — can opener needs cleaning. Corrected.

Hyatt Place, 4320 Cerrillos Road
Date inspected: May 7
Violations: One high-risk violation for improper holding — improper holding temperature of potentially hazardous food. Ham was at 59 degrees. Two moderate-risk violations, one for improper holding — no food-grade thermometer available to monitor food holding temperature; one for contaminated equipment — juice machine was not cleaned and sanitized.
Status of establishment: Approved no follow-up required.

Marble Brewing Tap Room, 60 E. San Francisco St.
Date inspected: May 14
Violations: Two moderate-risk violations, one for improper holding — no food-grade thermometer available to monitor food temperatures; one for contaminated equipment — improper three-step manual dishwashing.
Status of establishment: Approved, no follow-up required.

Mini Super Delicias, 4641 Airport Road
Date inspected: May 7
Violations: One high-risk violation for storage — cheese, deli meats were stored together with enclosed cheese. Also, in the walk-in cooler, produce was stored below raw meat items. Corrected. One moderate-risk violation for contaminated equipment — facility mixes dishwashing liquid with Clorox. Do not mix those two together.
Notes: Need to get another color cutting board to slice pre-cooked, ready-to-eat food products. Call NMED when purchased. The prep counter for cutting up raw meat and ready-to-eat meat products are so close together. Separate, assign one person to handle ready-to-eat meat, cheese only.

Pranzo Northern Italian, 540 Montezuma  
Date inspected: May 10
Violations: One high-risk  violation, for inadequate cooking/cooling and improper holding — some hot food items in coolers are not being cooled properly. Deep plastic buckets used. Some food items that will not be cooked were disposed of. Others will be reused in hot cooked foods. The two undercounter refrigeration units in front area of kitchen are not holding proper cold holding. Some ready-to-eat food items like dressings were disposed of. Two moderate-risk violations, one for contaminated equipment — pots and pans are not being sanitized. One for animals/vermin/openings — back door was left open. One low-risk violation for storage — bag of corn starch was stored on the floor, ice scoop was placed on filthy ice machine surface. Corrected.
Notes: Paper towel dispenser battery is dead. Replace. Facility is in the process of getting new three-door undercounter refrigeration unit. Will be arriving tomorrow from California.

Quail Run, 3701 Old Pecos Trail
Date inspected: May 9
Violations: One high-risk violation for improper holding — sliced tomatoes kept at 51 degrees, must be at 41 degrees or below. Corrected. Three moderate-risk violations, one for improper holding — no thermometer available inside ice cream freezer; one for contaminated equipment — ice cream scoop was not properly cleaned and sanitized after use; and one for storage — many unlabeled containers with unknown white substance in the kitchen.

Violations: One high-risk violation for improper holding — improper holding temperature of ham, beans. Corrected. Two moderate-risk violations, one for improper holding — food thermometers inaccurate, must be calibrated daily; one for contaminated equipment — improper three-sep dishwashing process.

Taqueria la Chabelita LLC, 4434 Airport Road
Date inspected: May 7
Violations: Two high-risk  violations, one for poor personal hygiene — hand sink is not in working order. No hot or cold running water; one for storage — raw meat in enclosed container stored next to jalapeño container. Corrected.
Notes: Adequate hot and cold running water is available. Cutting board looks very clean. Mobile unit looks very clean. Employee was wearing gloves when  handling ready-to-eat food.
Status of establishment: Approved, follow-up May 17.

Tia Sophia’s, 210 W. San Francisco St.
Date inspected: May 8
Violations: One high-risk violation for inadequate cooking/cooling — hot food items, beans, red and green chiles, are not being cooled properly. Left at room temperature, covered. Corrected. Facility must get the frozen bottle containers to help in cooling down food process. Faculty was taught how to cool hot food items down. Provide at least four of these frozen paddles. Three low-risk violations for floors/walls/ceilings — the floor tile in the refrigeration units room is broken. Fix. The floor in this room does not have a core base between floor and wall. The walls in this room are peeling. Seal and repaint.

Date inspected: May 7, complaint
Violations: One moderate-risk violation for contaminated equipment —  cutting boards in kitchen are stained. Sand or replace. One low-risk violation for floors/walls/ceilings — the wall underneath the dishwasher is broken.
Notes: A couple ate there on May 3, around 7:30 p.m. About two hours after leaving, they started vomiting, also had diarrhea all night and still feeling bad on May 4.
Complainant also called the facility regarding this illness. Food items eaten were macaroni and cheese and also a turkey sandwich. These are cooked/made to order. Ingredients in the mac and cheese: milk, cheese sauce. Ingredients for turkey sandwich: turkey, sourdough bread, provolone cheese, tomatoes, avocado and cumin. No employees were sick on this day. Number of people that had turkey sandwich: 10; mac and cheese: 9. Cannot determine how this illness happened.
