Date inspected: Jan. 13
Violations: Four high-risk violations. Ice scoops stored in ice machine, ice scoop has grime build up and needs to be stored in dish washable container. Pesticides stored in food preparation area. Dented cans mixed in with good stock, shall post a “do not used dented cans” sign in can storage area. Improper food storage. No date of preparation on salsa bar. All high-risk violations were corrected at time of inspection. One moderate-risk violation. Microwave is not ANSI or NSF approved. One low-risk violation. Employees have no hair restraints.

Bon Appetit, IAIA, 83A Van Nu Po Road
Date inspected: Jan. 17
Violations: Five high-risk violations. Wet wash cloths out of sanitizer bucket. No paper towels at hand washing station. No test strips/thermal labels for high temperature dish washing machine. Employee drinks, phones and cigarettes have the potential for cross contamination in food prep area. Potential for cross contamination of soiled meat string. Two moderate-risk violations. Top oven food equipment has grease build up. Employee medication stored on or over food prep/storage area. Three low-risk violations. Restroom door opens into dining/food prep area. Toilet paper roll not in dispenser. Employees have no hair restraints.

Inn of the Anasazi, 113 Washington Ave.
Date inspected: Jan. 21
Violations: Two moderate-risk violations. QUAT test strips were not available for two out of the three-compartment sink. Cutting boards are stained and discolored. Four low-risk violations. Boxes of food items were stored on the floor in walk in freezer and dry storage room. The seal between the wall and the three-compartment sink at the basement is eroding. Adequate lighting not available in the glass storage. Bulk storage was not labeled.

La Cocina de Clara, 227 Don Gaspar Ave.
Date inspected: Jan. 22
Violations: Three high-risk violations. No paper towels available for hand sink in kitchen, which was corrected at time of inspection. Hand sink in main kitchen was covered with tray of utensils. Food items in under counter refrigerator were not holding proper temperatures, Pico de Gallo, salsa and sour cream had to be dumped. Refrigerator must be fixed within 10 days of inspections. One moderate-risk violation. No paper towel dispensers, must be installed within 10 days. Two low-risk violations. Food permit posted expired in December 2013. Exhaust hood filters and under counter refrigerator are greasy and dusty.

Flying Star Cafe, 500 Market Street
Date inspected: Jan. 22
Violations: Refrigerator units is temporarily fixed for adequate temperatures, but facility must have permanent solution by Feb. 14.

Mariscos Costa Azul, 2895 Cerrillos Road
Date inspected: Jan. 23
Violations: Two moderate-risk violations. Freezer not NSF/ANSI approved. Non-food contact surface needs periodic cleaning. One low-risk violation. Inadequate lighting for food prep area and walk in area, also a light bulb is out in refrigeration unit.

Violations: Two high-risk violations. Hand towel dispenser not available at both hand sinks. Air gap not in place at dish machine, three-compartment sink and ice machine. Two moderate-risk violations. Unused equipment must be removed from kitchen. Light bulb out in kitchen prep area.

Violations: Three high-risk violations. Hand washing station has dirty dishes in it. No water at hand washing station due to the cold. Crock pot and slow cooker in use. All high-risk violations were corrected during the time of inspection. One moderate-risk violation. Food build up on storage equipment and grease build up on range hood. One low-risk violation. Employees have no hair restraints.

Date inspected: Jan. 17
Violations: Two high-risk violations. Hand sink had hot water turned off. Salad display refrigerator needs to be chilled. Both violations were corrected at time of inspection. Four moderate-risk violations. Freezers need to be replaced with commercial equipment. Cutting boards need replacing or resurfacing. Floors, walls and ceilings need to be cleaned and repaired. Walls and ceilings need painting.
