Hand drying paper towels are wet as they are not in a dispenser. Soap dispenser has soap build up and requires both hands to operate. Wet cloth not in sanitizer bucket, corrected on site. Cleansing chemicals are stored directly over three-compartment sink causing a cross contamination risk. Two moderate-risk violations. Microwave is not ANSI or NSF approved. All sides of food equipment has grease build up, vents and fans have dust and mold build up. One low-risk violation. Employees have hair restraints, nets, or hats.
