The following restaurant inspection reports were provided by the New Mexico Environment Department.

Violations: Two moderate-risk violations, one for poor personal hygiene — hand sink was removed from food prep area. Need to re-install in original food preparation area to make it convenient for employees to wash hands. Notify NMED when completed, within 30 days. One for animals/vermin/openings — back door has gap at bottom and side. Potential to allow insects and vermin access to facility. Need to seal.

Using hand sink to wash utensils., etc. Must be washed, rinsed and sanitized in three-compartment sink. Corrected. Manager to train staff. Sanitizer solution in bucket, dirty. Needs replacing ASAP. Keep at 50-200 ppm. One moderate-risk violation for contaminated equipment — three-compartment sink in back is inaccessible for proper washing, rinsing and sanitizing. Plan to make accessible for use by rearranging/changing location. Facility needs maintenance, thorough cleaning in back. Facility may get renovated, therefore insure any plans for renovation are submitted to NMED for review. One low-risk violation for poor personal hygiene — food prep employees must wear hair restraints while working in food prep area.

Date inspected: Jan. 8
Violations: One high-risk violation for inadequate cooking/cooling — beans in the walk-in cooler did not have adequate cold-holding temperature. Was 52 degrees. Corrected. Taken and put into an ice bath container. The process of cooling down food was taught to staff. One moderate-risk violation for contaminated equipment — the QAT sanitizing solution is not adequate at 100 ppm. One low-risk violation for administration — food permit posted has expired. Kitchen equipment looks clean, but proper cooling process must be followed.

Two for floors/walls/ceilings — base coring repair/replace several places; wall and ceilings have peeling paint, easy-to-clean surface needed. Opening to wall interiors. One for ventilation/lighting — inadequate moisture ventilation in warehouse area.

Date inspected: Jan. 11
Violations: Two high-risk violations for contaminated equipment — Not washing and sanitizing properly in three-compartment sink, washing sanitizing and rinsing. Corrected. Soda and Ziploc bags stored in toilet room on top shelf. refrigerator has broken handle, two steamer lids cracked. Corrected, repeat violation. Three moderate-risk violations, one for contaminated equipment — inadequate space for clean equipment, cutting board stored next to hand sink. One for poor personal hygiene — microwave above hand sink impedes access, move microwave to allow ease of access. One for animals/vermin/openings — vent in ceiling not protected with mesh or screen.

Violations: Three high-risk violations, one for improper holding — turkey at 46 degrees, ham at 46 degrees, roast beef at 47 degrees. Two for poor personal hygiene — wash area hand sink drain blocked/slow and hot water at 70 degrees. CAR written response required to address improper holding and poor personal hygiene violations, by
3 p.m. Jan. 17. One moderate-risk violation for contaminated equipment — prep table edge/seam has some particle accumulation. Three low-risk violations, one for storage — foil containers not stored inverted. Two for ventilation/lighting — inadequate light in ware wash area. Unprotected light in prep area.

Violations: One moderate-risk violation for contaminated equipment — cutting boards are stained and discolored. Sand or replace. Kitchen looks clean.

Violations: One moderate-risk violations, one for improper holding — McCall Reach-in refrigerator not holding proper cold holding, but food items in it were just made. These were immediately removed and placed in walk-in cooler. One low-risk violation for floors/walls/ceilings — some kitchen floor areas are messy. Clean floors more often.
Notes: Generally, kitchen is clean. All food items dated on when made and when they expire. Record on that day, 13 people had the same soup. No other person called in sick. According to facility manager, any leftover soups are tossed away. Soups are made fresh daily. Cannot determine where this illness came from. Facility operations looked OK.

Sour cream at 47 degrees, ham at 47 degrees and green chile at 46 degrees. Fix within three days. One moderate-risk violation for contaminated equipment — the long cutting board is stained with food items. Always clean and sanitize with bleach at the end of the day. Two low-risk violations, one for administration — the food permit posted has expired. One for floors/walls/ceilings — the floors under the three-compartment sink and dishwasher are cracked and peeling. Fix within three months.

Frozen ice paddles were placed on the ones that are not holding proper temps. One for contaminated equipment — some employees were touching ready-to-eat foods with bare hands. Corrected. One moderate-risk violation for animals/vermin/openings — the back door has an opening underneath it. Provide seal so insects do not come into facility. One low-risk violation for floors/walls/ceilings — the core base seal between the wall and floor is peeling off in the dry storage room area. Fix.
