Violations: One moderate-risk violation for improper holding — need thermometer inside refrigerator.

Notes: Staff wearing hairnets. Continue cleaning, maintenance ongoing. Received CAR letter. Good.

Violations: Seven high-risk violations, one for unsafe sources — refrigerated foods not dated. Two for contaminated equipment — serving utensils held in cup, non-running water; clean dishes/plates held on raw chicken contaminated cutting board. Corrected. Three for poor personal hygiene — Employees not washing hands at proper intervals or events. Using cloths to dry hands, touching floor and handling dishes. No hand sink in wash area. Prep area hand sink not easily accessible. Prep table impedes frequent access. Must be relocated or provide better access. One for storage — raw ground beef and raw chicken stored above ready-to-eat items. Corrected. Five moderate-risk violations, one for improper holding — not monitoring refrigerated cooler food temps. Two accurate digital thermometers on hand. Four violations for contaminated equipment — not testing sanitizer adequately. Test kit damaged; inadequate space for clean dishes and dirty dishes. Items held on floor; employee washing hands in three-compartment sink; all cutting boards discolored, scratched. Four low-risk violations, two for floors/walls/ceilings — toilet room door not closed. Held open during ware washing operations; opening in ceiling above cooler, 16”x16” for electrical. One violation for storage — dishwasher handling dirty dishes and clean dishes without washing hands between. One violation for ventilation/lighting — inadequate light in wash area, walk-in cooler.

The Burrito Company, 111 Washington Ave.

Date inspected: Dec. 28, follow-up

Violations: None

Notes: All temps IAW NMAC. Carne adovada at 38 degrees; ground beef at 160 degrees, chicken at 38 degrees. Personnel demonstrated proper hand washing. All refrigerated food items dated.

Recommendations: Remove toilet in back area or install new hand sink in ware wash area; Hire consultant to inspect weekly, for 52 weeks. Redesign facility to allow adequate space for cooling food, holding refrigerated items and submit plans for review after certifying that designer has designed to met NMAC 7.6.2. Maintain daily temperature logs every four hours and keep copies and submit originals each week, on Tuesday, to NMED. Design must address all violations cited in past five years. Submit plans for review. Required to have personnel food safety trained by Jan. 10. All food handlers must complete food safety test as administered by inspector. On Jan. 25, must call to schedule after training is completed and show proof of training.

Violations: Two high-risk violations for contaminated equipment — ice scoop laying on top of icemaker. Needs to be stored in container. Corrected. Sanitizer < 50 ppm. Corrected. One low-risk violation for animals/vermin/openings — screen door in back has large opening on bottom. Must be sealed to keep vermin/rodents out.

Violations: One high-risk violation for poor personal hygiene — no hand sink behind bar, not in immediate vicinity. Not sanitizing hard surfaces, no sanitizer ready for use at time of inspection. Solution mixed greater than 200 ppm, remixed at 100 ppm. Two moderate-risk violations for contaminated equipment — test kit color chart damaged with water, not usable. Dishwasher tested at 100 ppm. Inadequate space for dirty dishes. Two low-risk violations, one for floors/walls/ceilings — no base coring in prep area or ware wash area. One violation for ventilation/lighting — inadequate light in cooler.

Violations: One high-risk violation for plumbing/waste disposal — backed up sewer water flooding storage room floor with ice machine. Room closed off and equipment quarantined. Sewer unblocked and equipment and area disinfected and sanitized. Corrected. One moderate-risk violation for contaminated equipment — inadequate space for clean dishwashing racks. Held on floor and for dirty dishes. Four low-risk violations, one for animals/vermin and two for floors/walls/ceilings — walls and ceiling peeling paint, wall plaster missing and deteriorating in wash room area. One violation for ventilation/lighting — inadequate light in bar area, wash area and cooler.

Three low-risk violations, two for floors/walls/ceilings — floor and walls junctures not cored properly in prep area; walls in prep area have peeling paint, some particle accumulation. One violation for ventilation/lighting — inadequate light protection in tortilla prep area.

Mucho Gourmet Sandwich Shop, 1711-G Llano St.

Date inspected: Dec. 26, complaint

Violations: One high-risk violation for improper holding — turkey breast at 47 degrees in refrigerator, steak at 45 degrees. Corrected. Items put into sheet pans for rapid cooling. Two moderate-risk violations, one for improper holding — one inaccurate and one difficult-to-read thermometer on hand. Corrected, new one calibrated for use. One violation for contaminated equipment — sanitizer test kit not operable. Papers water damaged. Not testing. Corrected, trained operator and will purchase new kit. Five low-risk violations, three for floors/walls/ceilings — toilet room doors not self-closing; ceiling tile improper, not smooth, absorbable, discolored. Two violations for ventilation/lighting — inadequate lighting in prep area; dusty air exhaust vent in prep area. 

Notes: Must provide written plan to correct violations by Jan. 15. Complainant ate at Mucho Dec. 4 and had the turkey surprise. That evening, she was sick for more than an hour. Also had fever and chills.

Old Pecos Foods, food processor, 42 Glory Lane

Violations: One for light not being operable, inadequate light.

Notes: One product batch code not correctly recorded in logbook. Most provide letter to rework product on hand and correct logbook and also state if recall or public notice is needed.

Regal Santa Fe Stadium 14, 4051 Cerrillos Road

Date inspected: Dec. 31

Violations: Two high-risk violations, one for contaminated equipment — using disinfectant in lieu of proper approved sanitizer. Disinfectant not approved for use in food service. One violation for plumbing/waste disposal — plumbing ran along floor, making area below difficult to access and clean. One moderate-risk violation for contaminated equipment — inadequate space for dirty and clean dishes to properly drain and air dry. Drain board full of dirty dishes, clean dish area dirty from using it to clean oven racks. Cart provided for drain and air dry, not proper. Dishes held in pool of water. One low-risk violation for storage — single-serve articles stored on bottom shelf, unwrapped and open, face upward. Destroy exposed top ones, wrap remainder and store inverted.

Rooftop Pizzeria, 60 E. San Francisco St.

Date inspected: Dec. 26, complaint

Violations: One high-risk violation for poor personal hygiene — hand washing sink obstructed, not accessible for use. Corrected. Two moderate-risk violations, one for improper holding — thermometers inaccurate, temperature must be recorded and monitored. One for contaminated equipment — improper dishwashing process. Residual water inside containers.

Notes: Two diners ate at Rooftop Pizzeria on Dec. 18 and about two hours later, started feeling ill. Couple ordered blue corn pizza. Inspected, no problem regarding complaint found. Complaint closed.

Status of establishment: Approved, notify NMED by Jan. 3, with written report.

 

Yadi and Scotty’s Sidewalk Café

Date inspected: Dec. 27

Violations: None

Notes: Unit in storage, not operating. Mobile unit. Roasted corn and bottled beverages. No menu change. Will call for inspection before operating unit in 2013.
